29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    a.  On or about May 27, 2016, in the facility, employees were required to operate powered industrial vehicles and had not been provided with training including but not limited to formal instruction, practical training and an evaluation of the operators performance thereby exposing employee to struck by and crushing hazards.
