A)        on or about 06/01/2016 - On the 14th floor of 27-45 Jackson Avenue, Long Island City, NY      An employee was using an A-frame ladder as a straight ladder.  The ladder was leaned against the wall while he was painting the bathroom ceiling.      ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
