29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  a)  On or about May 31, 2016 at 490 Albert Way in Appleton, an employee was working from a bunk of plywood supported by the forks of a Gradall 534C material handler, which was boomed out while an employee was standing on the bunk of plywood.  The forklift was overloaded as the boom was extended causing the forklift to tip over, the load to slide off, and it catapulted the employee off of the forklift.
