Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly:  a)  On or about June 6, 2016, in the warehouse, the employer did not ensure to get the manufacturer's prior written approval to add the extension fork attachments to the Allis-Chalmers, Model #FP60-242PS with Serial #46258000.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  a)  On or about June 6, 2016, in the warehouse, the employer did not ensure to mark with plates, tags, or decals the addition of the extension fork attachment on the Allis-Chalmers, Model #FP60-242PS with Serial #46258000 to show the change of the approximate weight of the truck and attachment combination at maximum elevation with the load laterally centered.

29 CFR 1910.1200(f)(6)(ii): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  a)  On or about June 6, 2016, in the shop, employees used an unlabeled bottle of Windex Complete Glass & Multi Surface Cleaner, to clean the aquarium plastic tanks.  The container did not have a product identifier with words, pictures, symbols, or combination thereof that provided at least general information regarding the physical and health hazards of the chemical.  b) On or about June 6, 2016, in the greenhouse, employees used a unlabeled Activate 5.25% Institutional Bleach bottle, to sanitize the greenhouse tables.  The container did not have the correct product identifier with words, pictures, symbols, or combination thereof that provided at least general information regarding the physical and health hazards of the chemical.
