29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end.  On or about June 6, 2016 and at times prior thereto, the employer did not ensure the platform overhang did not extend more than 12 inches over the scaffold end frame and/or install guardrails which would block employee access to the cantilevered end, exposing the employees to a fall hazard.
