29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.    On or about June 16, 2016, at a worksite located at Lot 28 Royalwoods Drive, Hudson, OH 44236, during construction activities including but not limited to residential construction activities, employees were exposed to fall hazards approximately between 12 and 23.8 feet. The employer did not ensure the attachment point of the harness was located in the center of the wearer's back.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about June 16, 2016, at a worksite located at Lot 28 Royalwoods Drive, Hudson, OH 44236, during construction activities including but not limited to residential construction activities, employees were exposed to fall hazards approximately between 12 and 23.8 feet. The employer did not ensure a fall protection system/device was being used while employees were installing weatherproof material on a steep slope roof.    LT Contractors Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1104945, citation number 01, item number 002 and was affirmed as a final order on January 7, 2016, with respect to a workplace located at 33690 Retreat Drive, Medina, OH 44256.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    On or about June 16, 2016, at a worksite located at Lot 28 Royalwoods Drive, Hudson, OH 44236, during construction activities including but not limited to residential construction activities, employees were exposed to fall hazards approximately between 12 and 23.8 feet. The employer did not ensure portable ladders extended 3 feet above the upper landing or a grasping device was provided.    LT Contractors Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1104945, citation number 01, item number 003 and was affirmed as a final order on January 7, 2016, with respect to a workplace located at 33690 Retreat Drive, Medina, OH 44256.
