The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:  In the warehouse:  On or about June 23, 2016, employees were exposed to an ejection/crushing hazard, while operating a Nissan forklift (Model CF30LP) without being certified as trained in its safe use and operation.

29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the load approved by the building official was not marked on plates of approved design and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate:  In the warehouse:  On or about June 23, 2016, a load rating was not posted for a mezzanine used for storage to indicate how much weight could be distributed by pound per square foot.
