29 CFR 1910.212(a)(3)(ii): The point of operation of machines, whose operation exposes an employee to injury, was not guarded. The guarding device was not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, was not so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.     a) On or about June 22, 2016, at the above located workplace, employees were required to work with a Chicago Automatic Folder machine ; Model, Air Chicago; Serial Number, 47552. The back end of the folder machine did not have an effective guard. The employees were thereby exposed to amputation /crushing type injuries.

29 CFR 1910.264(d)(2)(i)(a): A safeguard, safety appliance, or device attached to, or forming an integral part of machinery was removed or made ineffective and was not replaced immediately upon the completion of such repairs or adjustments:    a)  On or about June 22, 2016, at the above located workplace, employees were required to work with a Chicago Automatic Folder machine; Model, Air Chicago; Serial Number, 47552. The back end of the folder machine did not have an effective guard and the interlock (automatic stopping switch) was disconnected. Employees were thereby exposed to amputation / crushing type injuries.
