29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.   a) On or about March 22, 2021, in the towel folding department, at folding machine #5, Air Chicago Model # 47551, the employer failed to ensure an energy control procedure was utilized for the control of hazardous energy when an employee was engaged in maintenance or servicing activities, thereby exposing employees to fractures, and/or amputation hazards.  Five Star Laundry  was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147 (c)(4)(i), which was contained in OSHA inspection number 1394782, citation number 1, item number 2 and was affirmed as a final order on September 24, 2019, with respect to a workplace located at 6000 Osceola Parkway, Kissimmee, FL 34746.

29 CFR  1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed.   a. On or about March 22, 2021, the employer had not performed annual periodic inspections to ensure that documented lockout/tagout procedures were being followed by employees and that the procedures were effective. On the towel folding machine #5 Air Chicago Model 47551, the maintenance mechanic performed a service and maintenance activity when removing a panel on the towel folding machine and fractured right hand thumb finger.  Five Star Laundry  was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147 (c)(6)(i), which was contained in OSHA inspection number 1394782, citation number 1, item number 3 and was affirmed as a final order on September 24, 2019, with respect to a workplace located at 6000 Osceola Parkway, Kissimmee, FL 34746.
