Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees performing overlashing and/or hog lashing duties in that employees were exposed to fall hazards:  (a) M/V Forest Trader: On or about June 22, 2016, and at times prior thereto, employees were working within three feet of the port-side edge. This exposed employees to a 20-foot fall hazard.  (b) M/V Forest Trader: On or about June 22, 2016, and at times prior thereto, employees would work at the edge of a log load while performing lashing operations. This exposed employees of up to a 25-foot fall hazard.
