29 CFR 1926.1053(b)(6): Ladders were not used only on stable and level surfaces unless secured to prevent accidental displacement: a) NorthStar Demolition and Remediation, Inc., 7401 East Martin Luther King Boulevard, Denver, CO 80237: On and before June 11, 2016 the employer allowed the use of a ladder on a surface that was not stable and level, and the ladder was not secured to prevent accidental displacement. An employee fell off a step ladder he had placed on top of a 4'' floor hole that was covered with poly material. This condition exposed a employee to a fall hazard.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) NorthStar Demolition and Remediation, Inc., 7401 East Martin Luther King Boulevard, Denver, CO 80237: On and before June 11, 2016 the employer did not ensure that each employee on the dance floor was protected from tripping in or stepping into or through a floor hole. An employee was working near a 4" floor hole, covered with poly material, when he was setting up his ladder. This condition exposed a employee to a tripping hazard.
