29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) Water line installation, State and Sterling streets - On or about July 13, 2016, two employees worked in a trench greater than 5 feet in depth installing an 8 inch water line without using any form of protective system.
