29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):      a)  Lakeside Quality Building Products, on or about 7/14/16:  An employee working from an elevated pallet was not protected from falling approximately 9 feet 10 inches to the ground.

29 CFR 1910.333(c)(3)(i)(A)(1): When an unqualified person was working in an elevated position near overhead lines with voltages to ground rated at 50 kV or below, the location was not such that the person and the longest conductive object he or she could contact came closer than 10 ft. (305 cm) to any unguarded, energized overhead line:       a)  Lakeside Quality Building Products, on or about 7/14/16:  Forklift was used to elevate an employee who was standing on a wooden pallet.  The mast of the forklift was within 10 feet of the electric service to the building.
