29 CFR 1910.179(j)(2)(iv): A monthly inspection, with certification, was not conducted of Hoist chains, including end connections, for excessive wear, twist, distorted links interfering with proper function, or stretch beyond manufacturer's recommendations.   a/ Meritex LLC, 160 Circle Drive North, Piscataway NJ site. - The Employer did not maintain written certification records of the monthly hoist chain inspection. The monthly certification record to include the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected.  Observed on/or about 06/29/2016.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, were not adequately guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a/ Meritex LLC, 160 Circle Drive North, Piscataway NJ site: Elevator #1(Hoist#1)Area. - The access door to the operating overhead conveyor system , caged area was not provided with a lock or chain and did not provide sufficient guarding, exposing employees to a struck by hazard by containers weighing 250lb or more being moved on an automated overhead conveyor system.  Observed on/or about 06/29/2016.
