29 CFR 1910.95(d)(3): Repeat employee exposure monitoring was not performed whenever a change in production, process, equipment or controls increases noise exposure:    Location: Mill works building  On or about 7/15/16, the employer did not conduct noise monitoring when a new process was added to the workplace  which included assembling wood crates using pneumatic staple guns. Employees were exposed to noise levels exceeding the action level 50% dose or equivalent sound level of 85 dBA for an 8 hour TWA.

29 CFR 1910.95(i)(2)(ii)(A): The employer did not ensure that hearing protectors are worn by any employee who is exposed to an 8-hour time-weighted average of 85 decibels or greater and has not yet had a baseline audiogram established pursuant to 29 CFR 1910.95(g)(5)(ii):      Location: Mill Works building  On or about 75/16, The employer did not ensure employees who were exposed to noise levels greater than 85 decibels wore hearing protectors prior to getting a baseline audiogram.
