29 CFR 1910.36(a)(3): An opening into an exit was not protected by a self-closing fire door that remains closed or automatically closes in an emergency upon the sounding of a fire alarm or employee alarm system. Each fire door, including its frame and hardware, must be listed or approved by a nationally recognized testing laboratory:   a) Broad St. exit stair tower: The latch mechanism on the basement fire door was damaged and not functional, as observed on or about July 26, 2016.  b) Broad St. and Hospital exit stair towers: The fire door listing and approval labels were obscured due to being painted over, as observed on or about July 26, 2016.  c) Hospital exit stair tower: Self-closing fire doors, located at the basement level, had been propped opened using wood wedges and carpeting, as observed on or about July 26, 2016.  Abatement Certification Required.

29 CFR 1910.36(g)(1): The ceiling of an exit route was not at least seven feet six inches (2.3 m) high and/or any projection from the ceiling reached a point less than six feet eight inches (2.0 m) from the floor:  a) Hospital exit stair tower: A radiator unit projected from the ceiling at a height of 6 feet 2 inches above the landing level, as observed on or about July 26, 2016.  Abatement Certification Required.

29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:  a) Facility wide: The employer did not provide all employees an educational program to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting, on or about July 26, 2016.  Abatement Note: By the date, the employer must either correct the alleged violation or implement a Fire Safety Policy as outlined in 29 CFR 1910.38(a)&(b), which includes the evacuation requirements of 29 CFR 1910.157(b).  Abatement Certification Required.
