29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  a.  On or about August 8, 2016, at the above addressed jobsite, employees were performing framing operations/erecting roof trusses without the use of ladders or other safe means of access. The employees were standing on and climbing the wall braces to access their work zone, thereby exposing the employees to fall hazards of approximately eight (8) feet.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stair rail system along each unprotected side or edge:  a.  On or about August 8, 2016, at the above addressed jobsite, employees performing framing operations in new construction were using a staircase with more than four (4) risers that was not equipped with at least one handrail or stair rails along the unprotected sides/edges, thereby exposing the employees to fall hazards of approximately eight (8) feet.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   a.  On or about August 8, 2016, at the above addressed jobsite, employees performing framing operations/installation of roof trusses near unprotected sides/edges were exposed to fall hazards of approximately twenty (20) feet.   b.  On or about August 8, 2016, at the above addressed jobsite, employees performing framing operations inside a new construction home were working near unprotected sides/edges, window and wall openings, thereby exposing the employees to fall hazards of approximately ten (10) feet.
