Employees were using a steel blade to scrape the material off the inside of the storage tank after washing the tank with the flammable liquid primer.

Forest City Technologies, Inc. located in Wellington, Ohio:  On or about July 22, 2016, the employer did not ensure employees using a flammable liquid primer to wash the residue from the inside of a storage tank located in Plant 4, bonded the dispensing container to the storage tank prior to dumping the flammable primer into the tank.    b. Forest City Technologies, Inc. located in Wellington, Ohio:  On or about July 22, 2016, the employer did not ensure employees dispensing a flammable liquid primer into a 5 gallon bucket, grounded the storage drum to the bucket prior to transferring the liquid.
