29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Forest City Technologies, Inc. located at 401 Magyar Street in Wellington, Ohio; On or about December 4, 2019 the employer did not ensure employees were protected while operating the DC-12, IC-2, and BC-3 FTS Coating Machines. Employees were exposed to caught in hazards in that the rotating index wheels are not guarded.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Forest City Technologies, Inc. located at 401 Magyar Street in Wellington, Ohio; On or about December 4, 2019 the employer did not ensure periodic audits of energy control procedures were completed at least annually. Set-Up employees and maintenance personnel are required to perform change overs and set-up activities on coating machines such as, but not limited to, P-14, A&C-6, and IC-1, which require energy control procedures that have not been reviewed for deficiencies.
