29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift: a) On or about May 28, 2016 and at times prior, an employee was exposed to fall hazards of approximately 30 feet high. The employee did not used a fall restraint system to prevent falls to a lower level while working in the basket of an aerial lift.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) On or about May 28, 2016 and at times prior, an employee was exposed to fall hazards of approximately 30 feet high while working inside the basket of an aerial lift. The employee did not have the knowledge and understanding of fall hazards using the boom lift and was not provided with scaffold training.
