29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.   a) On August 16, 2016, on the West side of the Nikko Construction site, electrical equipment powered by extension cords did not have ground fault protection, exposing employees to electrical hazards.

29 CFR 1926.405(a)(2)(ii)(J): Extension cord sets used with portable electric tools and appliances were not of the three-wire type:   a) On August 16, 2016, on the West side of the Nikko Resort construction site, electric blower pumps were powered by two-wire household type extension cords, exposing employees to electrical hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:   a) On August 16, 2016, on the west side of the Nikko construction site, several 110VAC motors did not have a strain relief device and outer insulation removed causing a transmission of tension to joints and/or terminals, exposing employees to electrical hazards.
