29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) On August 16, 2016, on the west side of the Nikko construction site, several 110VAC motors did not have a strain relief device and outer insulation removed causing a transmission of tension to joints and/or terminals, exposing employees to electrical hazards.
