29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or  more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems:  a. On August 19, 2016, on the 4:12 pitch tile roof, three employees applying mortar did not attach their harnesses to the anchors and ropes of the fall protection system installed on the roof.
