On or about January 16, 2020, on a jobsite located at 1927 Beach Avenue, Jacksonville, Florida, an employee suffered a fall resulting in a hospitalization and the employer did not report the work-related accident to OSHA within 24 hours.

On or about January 16, 2020, on a flat deck, employees were not trained to recognize the hazards associated with an in proper guard rail system, exposing them to fall hazards.
