29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:      On or about August 23, 2016, and times prior thereto, employees worked below other employees installing metal panels 18 feet above in a JLG Telescopic Boom Lift did not wear a hard hat for protection against falling objects.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    On or about August 23, 2016, and times prior thereto, employees were observed in a JLG 460SJ Telescopic Boom Lift without the use of personal fall restraints.  This practice created a condition which exposed employees to 18 ft. fall hazards.
