(a) On or about 8/23/2016, The Mount Sinai Hospital dba Mount Sinai Kidney Center at East River Plaza failed to keep and maintain a separate log of injuries and illnesses. The employer, instead, maintained a single log for the entire hospital system. (1) On 7/20/2016, a dialysis care technician sustained a lost-time injury, where a worksite-specific OSHA 300 log was not kept.
