An employee suffered an electric shock when he came in contact with energized (277 V) neutral wire. This condition was noted on or about April 8, 2026.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Employees perform servicing and maintenance activities on equipment to include, but not limited to, removal and replacement of electrical equipment which uses electrical energy as its source. Thus exposing the employees to potential start up from possible undocumented modifications. This condition was noted on or about April 23, 2026.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
