29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about at 8/25/16 located at 5940 Bradenton Rd., Alpharetta GA: Employees were exposed to eye injury, where employees were using a pneumatic nail gun without safety glasses while installing the rafters and braces on the roof.    Erick Esquivel- Reyes was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926. 102 (a)(1), which was contained in OSHA inspection number 1139179, citation number 1, item number 1, and was affirmed as a final order on 6/18/2016, with respect to a workplace located at 664 Bell Creek, Cumming, GA.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about at 8/25/16 located at 5940 Bradenton Rd., Alpharetta GA: Employees were installing rafters and braces on the roof while standing on the edges of a house that was under construction working at the edge, the employer did not ensure that fall protection was used by the employees while exposed to fall hazards of approximately up to 35 - feet and with a pitch of 8/12 & 10/12.   Erick Esquivel- Reyes was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926. 501 (b)(13), which was contained in OSHA inspection number 1139179, citation number 1, item number 2, and was affirmed as a final order on 6/8/2016, with respect to a workplace located at 664 Bell Creek, Cumming, GA.
