On or about August 26, 2016, at the above addressed jobsite, employees were required to use an A-Frame fiberglass ladder which was leaned against the side of the building to gain access to the first floor, thereby exposing the employee to the hazards associated with falls of six (6) foot.    b. On or about August 26, 2016, at the above addressed jobsite, an employee was working from an A-Frame fiberglass ladder which was leaned against the side of the building on the second floor, thereby exposing the employee to the hazards associated with falls of six (6) feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a.  On or about August 26, 2016, at the above addressed jobsite, employees performing residential construction operations on the roof level, over twenty-five (25) feet above the ground, were not protected from falling, thereby exposing employees to fall hazards.    b.  On or about August 26, 2016, at the above addressed jobsite, employees performing residential construction operations next to window openings on the second floor, seventeen (17) feet above the ground, were not protected from falling, thereby exposing the employees to fall hazards.    Zhuk Construction Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number1066694, citation number 1, item number 1 and was affirmed as a final order on July 27, 2015, with respect to a workplace located at 2640 W. Huron St., Chicago, IL.
