29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a) Maintenance shop, red water chiller, on or about September 13, 2016 and at times prior to, an extension cord was being used in lieu of fixed wiring to power the control unit for the red chiller, resulting in a fire and electrical shock hazard.
