29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a. North End Community Improvement Collaborative, Inc. dba Temp2Higher , at the worksite located at 849 Crawford Ave. in Mansfield, Ohio: The employer did not ensure that employees successfully completed a forklift operator training program containing elements in 29 CFR 1910.178(l) before being permitted to operate a forklift.  An employee(s) was assigned to operate a powered industrial vehicle(s) to load/unload trucks and to lift and maneuver materials such as, but not limited to, boxes and bales of metal.
