29 CFR 1926.304(i)(1): Each circular hand-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut.    (a) On or about July 11, 2016, for the employee who sustained serious injuries while operating a Bosch, Model No. 4100, Serial No. 30821 2012, table saw to rip wood window trim parts at 221 West Rosser Avenue, Bismarck, North Dakota.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four hours:  (a) On or about July 11, 2016, for the employee who sustained serious injuries while operating a Bosch, Model No. 4100, Serial No. 30821 2012, table saw to rip wood window trim parts where the ripsaw guard was not installed at 221 West Rosser Avenue, Bismarck, North Dakota.
