The siderails of the ladder only extended approx. 18 inches above the roof edge.    b) On or about 09/06/16, in the driveway on the east side of a house at 1145 Hertel Ave., Buffalo, NY:  Employees engaged in roofing work were exposed to a potential fall of approx. 20 ft. to the ground when an extension ladder was used to access the pik on a ladder jack scaffold. The siderails of the ladder only extended approx. 21 inches above the pik.    c) On or about 09/06/16, in the driveway on the west side of a house at 1145 Hertel Ave., Buffalo, NY:  Employees engaged in roofing work were exposed to a potential fall of approx. 20 ft. to the ground when an extension ladder was used to access the pik on a ladder jack scaffold. The siderails of the ladder only extended approx. 18 inches above the pik.       NO ABATEMENT DOCUMENTATION REQUIRED
