29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  Shop area - On or about September 13, 2016, employees were required to perform servicing on the Unipress shirt sleeves machine, Union washer, and other operating equipment The employer did not establish a written energy control (lockout/tagout) program consisting of energy control procedures, employee training and periodic inspections. The employees were thereby exposed to the hazards associated with the unexpected energization of equipment.

29 CFR 1910.303(g)(1)(i): Working space for equipment likely to require examination, adjustment, servicing, or maintenance while energized shall comply with the following dimensions, except as required or permitted elsewhere in this subpart:  Facility east - On or about September 13, 2016, in the facility employees are required to work near and around an electrical panel that was blocked by one 55 gallon barrel, two poles and other materials stored in front of it.   Employees were thereby exposed to hazards associated with blocked electric panels.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means:  Facility east - On or about September 13, 2016, the end of a flexible conduit wires coming from an electrical junction box were not completely guarded, thereby employees were exposed to the hazard of contacting electrical conductors.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.  Facility area - On or about September 13, 2016, employees required to work with various chemicals including, but not limited to Picrin, DF 2000 fluid, and NDT Sour had not been trained and provided information on the chemical hazards to which they were exposed to in the workplace.
