29 CFR 1910.26(c)(2)(iv): Ladder(s) were not maintained in a good usable condition at all times.    On or about September 15, 2016, the employer failed to ensure the portable 6 foot aluminum ladder, which is used in the main facility for accessing the top sections of the vessels being worked on, was in good stable condition prior to being used.  Specifically, the ladder had a significant bend in the second and third support rung up from the bottom on the rear and also did not have a max weight limit noted on the ladder anywhere.
