29 CFR 1926.451(b)(5)(ii):   The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:  a)  211 Eisenhower Parkway, Livingston, NJ.  Employees were exposed to falls of 17 feet to the ground below from the South end of the scaffold platform as the end of the platform extended over its support by 6 feet and no end guardrails blocked employees from accessing the cantilevered end.                          Violation observed on or about 08/30/2016.

29 CFR 1926.451(b)(10): Scaffold components manufactured by different manufacturers were modified in order to intermix them and a competent person did not determine that the resulting scaffold was structurally sound:  a)  211 Eisenhower Parkway, Livingston, NJ.  Employees were exposed to falls of 17 feet to the ground below as the working platform of the scaffold was made from one section of a Werner extension ladder with a plank placed over the rungs.   Alum-a-pole scaffolding systems are not designed to be used with ladders being used as working platforms and a competent  person could not show that this scaffold was structurally sound.                          Violation observed on or about 08/30/2016.
