29 CFR 1926.451(b)(2) Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, the scaffold platform and walkway was not at least 18 inches (46 cm) wide:     a) 678 Cranbury Road, East Brunswick, NJ 08816. The employer allowed his employee to work on and from the forks of a Rough Terrain Lull/Forklift Truck when the forks aka scaffold platform and walkway was not at least 18 inches wide exposing the employee to fall hazards.     Occurred on 09/21/16.

29 CFR 1926.451(g)(1): Each employee on scaffolds more than 10 feet high were not protected from falling to the lower level:  a) 678 Cranbury Road, East Brunswick, NJ 08816. The employer allowed his employee to work on and from the forks of a Lull with no fall protection system in place exposing the employee to fall hazards in that the employee was not anchored to the mast of the Lull.   Occurred on 09/21/16.
