The employer installed shop made attachments on the Mitsubishi FD40 and Mitsubishi FG25 forklifts. The attachments were approximately ninety one (91) inches in length. The employer added the extensions for the operators to move over-sized and bulky loads. Employees were exposed to the hazard of being struck-by the load or forklift.

The employees drill various metals. Employees were exposed to the hazard of being caught-in the drill press. c) On or about October 5, 2016, at the Turn Master metal lathe, the lathe chuck was not guarded. The employees machine various parts on the lathe. Employees were exposed to the hazard of being caught-in the turning lathe.
