29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about October 5, 2016, at the Starfire plasma cutting table, there were no lockout tagout procedures developed. Employees clean out the metal cuttings and clean out the tracks. Employees exposed to such hazards as the unexpected start up of the cutting table, electrical, pneumatic  and thermal energies.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  On or about October 5, 2016, the employer failed to ensure that any modifications or additions had the manufacturers written approval. The employer installed shop made attachments on the Mitsubishi FD40 and Mitsubishi FG25 forklifts. The attachments were approximately ninety one (91) inches in length. The employer added the extensions for the operators to move over-sized and bulky loads. Employees were exposed to the hazard of being struck-by the load or forklift.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about October 5, 2016, at the Gator shear, the shear was not guarded on the back side. The employees cut various size metals. Employees were exposed to an amputation hazard.   b) On or about October 5, 2016, at the Ellis variable speed drill, the speed drill was not guarded or shielded. The employees drill various metals. Employees were exposed to the hazard of being caught-in the drill press.  c) On or about October 5, 2016, at the Turn Master metal lathe, the lathe chuck was not guarded. The employees machine various parts on the lathe. Employees were exposed to the hazard of being caught-in the turning lathe.
