29 CFR 1910.95(c)(1):  A continuing, effective Hearing Conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employee noise exposures equaled or exceeded an 8-hour Time-Weighted Average sound level (TWA) of 85 decibels (dBA).    An effective hearing conservation program which included noise monitoring, audiometric testing of employees and training of employees as detailed in the standard was not instituted for the following employees:    a) two employees working in the molding department were exposed to a recorded personal noise dosimeter sound level of 91 decibels during the 480 minute sampling period on 11/09/2016.    b) a production tech working near 4600 arm 1 was exposed to a recorded personal noise dosimeter sound level of 86 decibels during the 480 minute sampling period on 11/09/2016.

29 CFR 1910.120(q)(1): The employer did not develop and implement an emergency response plan to handle anticipated emergencies prior to commencement of emergency response operations.  On or about 10/06/2016 employees were exposed to isocyanate chemical hazards in that the tote containing Autofroth 9300A Isocyanate leaked onto the concrete floor in the production area.  The isocyanate tote was located in the foaming area and attached to the Cannon Foaming machine.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.   Employees working in the production area were exposed to hazardous chemicals including isocyanates in that employees were not trained on the chemicals they worked with in their work area.
