29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Production Area The employer did not develop energy control procedures for use during tool/mold change for Thermoformer machines, thereby exposing employees to amputation hazards. On 10/17/16, an employee changing mold on Thermoformer #3 (S/N 96077022) suffered partial amputation of fingers on right hand when pressure box fell, crushing employees hand. Condition occurred on or about 10/18/2016.
