The employer is failing to protect employees from eye hazards from flying particles.  This was most recently observed at 6830 Ashbrook Dr, Lincoln, NE, 68526 where employees of Erlin Trucios were observed using a pneumatic nail gun and were not using eye protection.

The most recent occurrence of this was observed at 6830 Ashbrook Dr., Lincoln NE 68516 where employees were observed installing composite asphalt shingles on a roof that was approximately 30'in height and were not utilizing fall protection.

This was most recently observed at 6830 Ashbrook Dr., Lincoln NE 68516 where employees were installing roofing materials on a roof that was approximately 30' above ground.  Employees interviewed admitted they had not received any fall protection training.

This was most recently observed at 6830 Ashbrook Dr, Lincoln, NE, 68526, where employees of Erlin Turcios were observed utilizing an extension ladder to access a roof approximately 30' above the ground, and their ladder only extended approximately 2' above the upper landing surface.
