29 CFR 1926.652(d)(2): Manufactured materials and equipment used for protective systems were not used and maintained in a manner that would have prevented employee exposure to hazards.   On or about October 20, 2016 employees were allowed to work inside of a trench box that was not used in the manner intended by manufacturer, by performing the following activities:   a. The lower spreader was removed from the west side of the trench box.   b. A spreader was cut to fit in at the center of the trench box and welded into place.    c. Two (2") diameter pins were not placed in the spreader and the collar connection had been removed.   Employees were exposed to the hazards associated with a trench box failure.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
