29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about October 18, 2018 and at times prior thereto, the employer did not ensure employee(s) use proper eye or face protection while connecting hoses and pumping brine water from a truck into tanks.  This condition exposed employee to eye injuries.

29 CFR 1904.40(a):  Basic requirement. When an authorized government representative asks for the records you keep under Part 1904, you must provide copies of the records within four (4) business hours.  On or about October 26, 2016 and at times prior thereto, the employer did not provide OSHA 300 logs within four business hours when asked by the Compliance Officer.
