29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. On or about October 25, 2016, employees were exposed to fall hazards when unhooking the lanyard from the D-ring, while installing felt paper at the edge of the roof on Building 5. b. On or about October 25, 2016, employee with no fall protection was exposed to fall hazards, when working on another lower level roof, while installing felt paper on Building 5.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds per employee attached, or were not designed, installed and used: Construction site, Building #5: On or about October 25 and at times prior thereto, two employees were exposed to fall hazards, while both were connected to the same Guardian anchor as part of their fall protection systems, when the anchor was only rated for 5,000 pounds.
