29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At the jobsite located at 8722 82nd Terrace Overland Park KS: Employees engaged in residential roofing activities from a 4:12 pitched roof were exposed to fall hazards of approximately 9 feet from the eave of the residence to the ground below in that fall protection was not utilized.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the jobsite located at 8722 W 82nd St. Overland Park KS:  Employees engaged in residential roofing activities from a 4:12 pitch roof were exposed to fall hazards of approximately 9 feet from the eave of the residence to the ground below in that the ladder used to access the roof was not extended at least 3 feet above the eave and was not secured to the structure
