29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours:  At the jobsite located at 4810 W 101st Terrace, Overland Park, KS 66207:  A work-related incident occurred on September 7th, 2016, resulted in an in-patient hospitalization and was not reported to OSHA within 24 hours.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  Employees engaged in sanding activities voluntarily using filtering face piece respirators were exposed to respiratory hazards in that information contained in Appendix D was not provided.
