29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) During September through November of 2016, at the facility where employees were exposed to amputation and caught-in hazards when performing service and/or maintenance tasks on equipment such as, but not limited to, the Wadkin GD5 molder and the Powermatic planer without utilizing the documented procedures for controlling hazardous energy. b) During October and November of 2016, at the facility where employees were exposed to amputation and caught-in hazards when performing service and/or maintenance tasks on equipment such as, but not limited to, Saw Stop table saws and the Extrema up-cut saw without developing, documenting, and utilizing procedures for controlling hazardous energy.
