29 CFR 1910.303(b)(7)(iv): Electric equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.    a)  On November 10, 2016, the employer did not ensure that the conduit on the conductor used to power parts of the extruder were attached to the electrical box.  Employees were exposed to electrical hazards while operating the line 2 extruder.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions:  a)  On November 10, 2016, the employer did not ensure that the opening where conductors entered the metal electrical box for the line 2 extruder were effectively closed and protected the conductors from abrasion.  Operators were exposed to electrical shock hazards.

29 CFR 1910.212(a)(1) One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by in going nip points:     a)  On November 3, 2016, the employer did not ensure that guarding was in good condition and functioning properly while employees were performing screen cleaning tasks on the line 2 extruder exposing them to amputation injuries from a nip point.
