29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    (a) Jobsite, Buncher Commerce Park, Avenue B, Suite 203A, Youngwood, PA:  On or about November 15, 2016 employees were operating the Clark and Mitsubishi powered industrial trucks and the employer had not provided training and evaluation to the employees to ensure they were competent to operate the equipment safely.
