29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59)  At the residential construction site located at or near 1301 NE 79th Street in Kansas City, MO:  Employees engaged in residential roofing activities were exposed to health hazards from dermal, ingestion, and inhalation exposure to hazardous chemicals.  The employer had not developed or implemented a written hazard communication program to ensure that employees were aware of the hazards associated with the chemicals with which they work.  Hazardous materials in use at jobsite included gasoline.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  At the jobsite located at 1301 NE 79th St in Kansas City, MO:  Employees engaged in residential roofing activities were exposed to an electrical shock hazard. The Chicago Electrical Powertools circular saw in use by employees did not have a ground pin to ensure the path to ground was permanent, continuous and effective.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the residential construction site located at or near 1301 NE 79th Street in Kansas City, MO: Employees were exposed to injury from an 18 foot fall hazard while accessing and exiting the roof.  The ladder that provided access to the roof on the front of the house did not extend 3 feet above the eave.
