29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  On November 30, 2016 at 1215 Prospect Avenue in Wausau during tear off of roof shingles, an employee was utilizing an extension ladder to access/egress the roof surface at 15 ft. above surface level without grab rails extending at least 3 ft. above the upper landing surface.

29 CFR 1926.1053(b)(9): The area around the top and/or bottom of (a) ladder(s) was not kept clear:   a)  On November 30, 2016 at 1215 Prospect Avenue in Wausau during tear off of roof shingles, an employee was utilizing an extension ladder to access/egress the roof surface with the base of the ladder within 1 ft. of a trash trailer, tarp, and work vehicle.

29 CFR 1926.1053(b)(10): The tops of a non-self-supporting ladder with more than one support attachment were not placed with the two rails supported equally:  a)  On November 30, 2016 at 1215 Prospect Avenue in Wausau during tear off of roof shingles, an employee was utilizing an extension ladder to access/egress the roof surface with one side rail supporting the ladder.  The top of the ladder was positioned over a corner in the roof eave edge with one side rail not used to support the ladder.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On November 30, 2016 at 1215 Prospect Avenue in Wausau during tear off of roof shingles, an employee was working on the roof surface with an eave edge 15 ft. above surface level without utilizing fall protection equipment.  The Superior Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1064213, citation number 01, item number 002 and was affirmed as a final order on July 13, 2015, with respect to a workplace located at 123 Pier Street; Merrill, WI  54452.
